   Do you acknowledge that our events benefit a charity and you will be
         asked to pay a donation to the charity on top of your exhibitor
          booth space fee? This fee will be told to you once approved.



   Do you acknowledge that you must submit photos along with this form
    to be juried into our shows? You are not an approved exhibitor until
     your products are approved and you receive an Official Entry Form
        directly from us. (You may not receive copied forms from your friends.)
